In addition to the completed form, the following information will be required:

  1. One copy of the original debit memo (stating debit reason - include all pages the Airline has sent to your agency)

  2. One copy of the Apollo-issued ticket  (the Agent copy, not the one that comes with the debit memo)

  3. The "ticketing" agency's Apollo pseudo city code

  4. Agency contact (Name, address, and pseudo for correspondence)
